Determine where patient will be seen utilizing Memorandum of Understanding (MOUs), MTF
MindLeaf with its partners provides the following services to MTF/Hospitals/Physicians:

Coordinates referral management functions and ensures team members receive information in a timely and efficient manner to facilitate continuity of patient care.

Coordinates specialty care referrals.

Reviews specialty care referrals for appropriateness and administrative and completeness.

Makes specialty care appointments.

Assists in providing patients specialty care referral information/instructions.

Serves as functional expert in the area of health benefits.
